Victoria was starved and tortured to death by her great aunt, and the woman s boyfriend, despite being known to four London boroughs, two hospitals, two police child protection teams and the NSPCC. The injuries which eight year old Victoria suffered were described by a home office pathologist as the worst he had seen. As on earlier occasions when the system has failed, the mistakes were multiple and spread among several agencies: a consultant paediatrician who misdiagnosed cigarette burns as scabies; social services ignoring separate warnings by a relative and a registered childminder about serious signs of abuse; and an almost unbelievable decision by a police officer and social worker to return Victoria, back into the hands of her abusers, from a hospital where doctors had identified abuse.
