After a community assessment, Mary was provided with a frozen meal to be reheated at dinner time and some adaptations were made to her house. For a year, with Avril s support, things seemed fine; however, Mary s memory began deteriorating and she started having problems with food preparation and managing personal care. After a fall and hospital stay, health care staff did not believe that Mary could be supported safely at home. Despite the risks, Mary and Avril strongly disagreed and, in partnership with social services, looked at putting together a care package. adapted from: http://www.communitycare.co.uk/Articles/2006/02/24/52940/dont-patronise-me.htm (a) Explain the following terms: Advocate _________________________________________________________ _________________________________________________________ _________________________________________________________ ______________________________________________________ [2] Empowerment _________________________________________________________ _________________________________________________________ _________________________________________________________ ______________________________________________________ [2] (b) Write down four stages of the care planning cycle. 1. ____________________________________________________ [1] 2. ____________________________________________________ [1] 3. ____________________________________________________ [1] 4. ____________________________________________________ [1] 5205 2 Examiner Only Marks Remark (c) Explain two different ways each of the following services may contribute to Mary s care.
