For Examiner s use only Question Number 1 2 3 Total Marks A3H51W9 4197 Marks 1 Jan Cerowski, aged 87, lives alone. Since Mr Cerowski s wife died he has become a recluse. A neighbour helps with shopping and errands. Due to his poor mobility, SSAFA, the national charity helping serving and ex-service personnel, referred Mr Cerowski to his GP. He had repeatedly fallen while trying to get out of bed. A district nurse visited and found Mr Cerowski s mobility was poor he used a broken broom handle to get around reinforcing his social isolation. He had not bathed for some time as climbing the stairs and getting in and out of the bath were unmanageable for him. He often eats standing up in the kitchen because he struggles to carry his food, hold his stick and walk at the same time. Mr Cerowski has poor hearing and now tends to speak in his native tongue, as he becomes more forgetful. Her flat has been adapted and she manages most tasks independently. She receives a package of care to help with household chores, to take her shopping and on outings and to attend a day centre. She is picked up by bus twice a week. Phoebe has repeatedly complained about the level of service she receives. She feels she is a low priority because she can manage her own personal care. Frequently new and temporary staff are sent and they are often late. Sometimes no staff at all turn up. She is convinced that because she has complained so often, they are providing a sub-standard service as a punishment . Phoebe has said that she wants to learn to drive so she does not have to be dependent upon anybody to get her about, and she wants the Trust to pay. They say they will not, even though Phoebe argues that if she could drive she would need less support and thus save the Trust money in the long term. She is also bored at the day centre and wants to do different things, especially gardening and carpentry; but the centre only offers very basic courses and Phoebe wants to excel.
