Joan had a stroke nine months ago which has left her disabled. She is paralysed down one side and her speech is also affected. She was discharged home seven months ago and now attends a physiotherapist at the hospital every fortnight. Joan has weekly physiotherapy sessions at home, which she pays for privately. A privately contracted domiciliary worker attends Joan every day to help with house work, cook meals and help her with personal care. We believe it is the responsibility of our services to adapt to disabled people s needs. The disabling factor is not the individual s physical limitations but rather the interaction of the wider society with the individual. For example . . . the reason a partially sighted person is unable to read a printed document is not because of their lack of visual acuity but because the document was not supplied in an appropriate format. Clients needs are determined as a result of the assessment, which is often carried out by a social worker, and a care plan is drawn up to meet the needs identified. Clients are encouraged to actively participate in this process and where this is not possible, advocacy is encouraged. Many clients are categorised as a result of the care planning process depending on their specific needs. The main aim of care planning is to ensure clients needs are met in the best possible way using a range of support services provided by organisations and individuals from within the mixed economy of care.
