For the past nine months he has been cared for by his wife, Amanda. He suffers long bouts of foregetfulness and rarely speaks or interacts with anyone. Paul, their son, lives nearby and he visits regularly with his two children. Mr Johnston recently fell and fractured his leg and is in hospital. His wife told staff that she has been finding it difficult to care for her husband. Prior to his admission to hospital he was not sleeping and would not eat the food his wife prepared. It has been agreed at the multi-disciplinary meeting that an occupational therapist will assess Mr Johnston s needs for returning home. A social worker has agreed to draw up a care plan and a home care worker will call twice a day to help Mrs Johnston to get her husband up in the morning and to bed at night. Two years later his wife and son joined him. Their son, Eduards, aged 18, has Down s Syndrome. He attends a day centre two days a week and goes on outings with the local Gateway club, which he really enjoys. It has helped him to feel welcomed by the local community. Mr Piebalgs now speaks good English, but his wife is finding it much harder to learn the language although she can generally understand it when people talk to her. Yesterday Mrs Piebalgs went to collect her son from the day centre. When she went into the activity room she saw two care workers watching her son playing pool. She realized they were talking about Eduards. They should not be giving places at the centre to foreigners, sure they haven t paid taxes. He s taking up a place . . . He should go back to where he came from, he doesn t belong here . . . sure look at the state of him, he can t even speak English, and sure what s the point of teaching him English. They walked away laughing and mimicking his accent. Mrs Piebalgs was very upset and later that night she discussed the situation with her husband. This is in line with the strategic vision described in A Healthier Future and the objectives set out in the Department s Public Service Agreement. The key aim is to support an increasing number of people to live independent lives, preferably in their own homes. To do this, the HPSS needs to develop effective alternatives to hospital care, which are designed to reduce inappropriate admissions and unnecessary lengths of stay. There also needs to be a strong focus on rehabilitation in tandem with assessment of long term care needs to avoid unnecessary reliance on residential and nursing home care.
